We understand you have many choices for your medical care and we are happy you have selected our office. We are dedicated to providing high quality medical care and service. We hope the following information assists you as you prepare for your visit.
Monday thru Friday 8am to 5pm (we are closed for lunch from 12pm-1pm).
We see patients Monday thru Friday and will work to accommodate your schedule.
We respect your time and do our best to see our patient's as close to their appointment time as possible. On occassion we may even be able to see you early. If we are running behind, we will try to keep you informed and we appreciate your understanding and patience.
New Patient Appointments
For your first visit to our office, please arrive approximately 20 minutes early. This will allow time for you to complete the registration process.
Log into our secure patient portal below. From here you may print out our pre-registration information in preparation for your visit. You may send us a secure message, request your next appointment, request a medication refill, view your lab results, and make payments.
If you do not wish to sign up for the patient portal, here is a link to our New Patient Forms (PDF File) and information for your review (PDF File). Print the forms, complete them in the privacy of your own home or office and bring them to your first appointment. You may also fax them to us prior to your appointment at (520) 888.2698 or mail them to us at 7436 N La Cholla Blvd, Tucson, AZ 85741. Getting your information to us prior to your appointment will allow faster registration on the day of your visit.
Accepted Forms of Payment
We accept Visa, MasterCard, Discover, American Express, cash and checks.
We have recently added Care Credit as an accepted form of payment for those patient's wishing a financing option. Select the button below to complete the application.
Our physicians are participating providers for United Healthcare and their associated plans, Blue Cross Blue Shield, Healthnet of Arizona, Pacificare, Secure Horizons, Cigna and Medicare. If you will be utilizing insurance for your visit, please remember to bring a photo ID, your insurance card, your referral and co-pay.
Our physicians prefer that you request your medication refills at the time of your office visit or during our normal business hours (Monday thru Friday, 8am to 5pm).

Please complete the above Medical Records Release form and return to our office. We will try to get this taken care as soon as possible for you. We request a minimum of 48 hours. Medical records must be picked up from our office and a photo ID may be required.
